Very obvious: Testicle separated from spermatic cord would atrophy (die & disappear). You would likely have a huge scrotal swelling from bleeding if cord broke. Thus could easily be diagnosed by physical. One testicle usually hangs a little lower than the other & significantly lower in some. No cause for concern if both testes are of similar size.
