Call the Dr: You are about in that window. Recommend calling the Surgeon / on-call number and discussing options with him/her. If no response after one hour (and calling back to re-page after 30 minutes while waiting), then go to local ER for hydration. 3yo does not have a big reserve. May just need better pain control to overcome, but don't wait much longer.
