ER visit: Well worth a trip to the ER. Could be abdominal process irritating you diaphragm. That would explain shoulder pain - there is a nerve that refers to the shoulder from the diaphragm

Go to ED: Sounds like you are quite sick since yesterday. You should go to ED to check out the possible cause of acute abdominal disease and receive proper treatment. Don't wait until Monday.
