Best guess: Most experts feel it came from central Africa as a cross over from a similar disease in possibly chimpanzees. The central africans trap and eat these relatives of humans and thereby expose themselves to whatever diseases they might carry. Once a human got it, modern transportation and social behavior guaranteed a wide spread.
