How to tell the difference between early paget's disease in the nipple and dermatitis or eczema?

Biopsy: There is Pagets disease of bone and Pagets of the nipple. The latter is carcinoma pf the terminal ducts of the breast which is the nipple. Here the nipple is ulcerated and the process extends to the areola. Biopsy should be performed to make the diagnosis.
