Fever & Sore Throat: This could be caused by many things other than strep. Many viruses can cause sore throat and fever that last a week or longer, including adenovirus, many enteroviruses(like coxsackie virus), and even influenza and related viruses. Other common ones that can look for all the world like strep throat on a physical exam are Epstein-Barr virus and CMV, both of which cause mono-like illnesses.

Virus,other bacteria: First, if a throat culture was not done, there is a slim chance it still could be strep. The rapid test misses 1-2 out of every 100 tested. Would ask for full throat culture to see if any other bacteria is the culprit. Also consider viruses like EBV (the cause of mono, in kids can cause intense sore throat), influenza (still a few cases still going around), or other viruses-depends on age of child
