Lump: Could be an early abscess with your history i'd recommend seeing a doc to be sure.

Need to see a Doctor: A tender swelling in the skin is often due to what is commonly referred to as a skin abscess or "boil". This is due to an bacterial infection in the skin or deeper. Sometimes it is due to an infected hair follicle. Warm soaks may cause it to "come to a head" and drain thus resolving the infection. But if the tender swelling persists or enlarges it may need to be lanced by your doctor.
