Examination: You need to be seen by 2 practitioners. You need to see a wisdom tooth expert, an Oral Surgeon, to determine vitality of 3rd molar (Physicians are not trained to do that). You need to see your General Physician or an ENT to evaluate your sinuses for infection (Not a Dentist's prerogative). You've gone as far as you can on the computer. Now go get examined/imaged to determine appropriate treatment.
