Yes: Though foreign objects under the nail usually come out in time they can cause a nasty infection and nail deformities if ignored. Removing them is quick and not too involved (unless there is already an infection or the nail is seriously injured) and can be done your doctor's office under local anesthesia.

Yes: The longer it sits there the higher the chance of getting an infection. Once you get an infection, the infection can spread to various places including the blood.
