No one knows: May be an issue in the "wiring" of the gut. Sometimes occurs after a GI infection: viral, bacterial, parasitic. Can be associated with things like migraine, anxiety, female sex, hormonal flux and sensitivity, family history.
Can have food, emotional, hormonal triggers.
