Bleeding: It may be nothing but you need a gyn exam to rule out pathology of dysfunctional uterine bleeding.

Many things: If your cycles are normally regular, this could be due to a small growth (polyp) inside the uterus or fibroid tumors(a benign condition). There could be other causes. You need a good exam if this continues. If it happens only one cycle and never again you could just wait.
