Possible irritation: Sometimes drool, food remnants, irritation from pacifiers/fingers/toys, or even wipes, creams or lotions near the mouth can irritate the sensitive delicate skin around the mouth of a child. Try your best (i know its hard) to keep that area dry; try a dap of aquaphor around the mouth to prevent irritation.
