Needs to be seen: Most likely not appendicitis, but cannot rule it out unless there is a current workup. Enlarged ovary should not cause fever (normally). Have this worked up, but please let the er doc know what previous workup has been done. And go to the same hospital where the enlarged ovary was diagnosed so they can refer to any labs/studies done.

Ruptured cyst: Have to be careful cause these symptoms can be due to appendicitis. However, rlq with a known ovarian cyst, throbbing/dull aching pain and sudden vomiting is most closely associated with an ovarian cyst that ruptured. These can be very painful and mimic the symptoms of appendicitis. If your fevers don't resolve, need to go to the er asap cause appendicitis is then more likely.
