Psychiatric eval.: Obsessive compulsive disorder involves unrealistic thoughts/ fear that the person may or may not realize is unreasonable ;/or repetitive behaviors. Anxiety symptoms can include feeling nervous or irritable, muscle tension (especially in neck, shoulders ; back), poor quality or minimal sleep, lack of concentration ; easy fatiguability. Lump in throat or butterfly in stomach sensation can occur.

Exam!: There can be various causes for feeling nervous keeping in mind we each have individual biochemical and psychodynamic makeups. A more complete history and some basic medical lab studies which include thyroid hormone testing would be needed for a start to begin to properly diagnose your condition. I than suggest supportive counseling to help deal with whatever your problem may turn out to be.
