Depends: If a patient has active cancer elsewhere and develops multiple new masses in the brain, most times we assume it's the same thing and do not need a biopsy. If you mean primary brain cancer, sometimes the MRI will be very convincing but as a rule you need to get in there to make the specific diagnosis and attempt to remove it if possible.
