If a curious person is concerned about being clandestinely poked with a needle, and constantly thinks about things like that, then the answer is, "of course you always know if somebody pokes you." if the person is easy-going and never thinks about those things, then the answer is, "possibly someone with a tiny needle can poke you without your knowing.".
