No: Pcos doesn't inherently cause pain, it is a metabolic disorder causing abnormal periods (which may be painful) but the ovaries are not painful. The cyst term is a misnomer...The patient does not actually have cysts, the term describes the appearance of the follicles (which contain the eggs) which are plentiful & stagnant.
