Looks like sacrum : A transitional L5 vertebrae is a fairly common finding when reviewing lumbar spine x-rays. It basically means that your L5 vertebrae has characteristics of s1( or the sacrum). Sometimes referred to as a "sacralized l5"... Nothing to be worried about...!
