Many possibilities: Your condition could be as a result of many different things. A thorough history and physical exam and possibly radiology exams such as an x-ray or an MRI are needed to determine the exact cause. Causes of pain may include: tendon instability, trauma (fracture, sprain, arthritis), musculoskeletal or a biomechanical abnormality.

Tendon issues: The most common thing that fits what you're describing is perineal tendon subluxation. If you put your hand over the lateral malleolus and move your foot you can feel the tendons snapping where they pass around a groove in the bone. This is usually not a problem but if it does cause you pain there are surgical procedures to correct it.
