Several: Add/adhd, chronic pain, depression, and dementia can all lead to severe disorganization that interferes with a person's life. There is clutter around, and they may try to organize it but don't have the energy or time. Compulsive hoarding can look a little similar, but is an anxiety disorder leading to compulsive buying or bringing unneeded items into the home -- which they then can't throw away.
