Heart problems: Heart problems are not common in teens and 20's.

Congnital heart dise: Teens and 20s do not normally have heart problems like coronary artery disease and cardiomyopathy etc. But some who are born with congenital heart disease even if cured by surgery may still have residual disease and complications, rarely they can have viral infections causing complications like cardiomyopathy.And occasionally rheumatic heart disease.These are not that common but still seen.
