Probably smoking: Although there hasn't been an actual study measuring lifespan reduction head-to-head between these 2 directly, my educated guess is that smoking is a greater health risk. Smoking not only risks developing cancer, but also causes lung disease all by itself. A little smile first: We have studies to show that smoking does shorten the lifespan of a person even without cancer in general. On the other hand, I am not sure there is study that would show anxiety alone, without the person doing things as a consequence of the anxiety, shortens life. We all have some degree of anxiety, that's the drive in us. A little bit too much should be taken care of.
