See below: 1.01 million cases were reported in 1978, the highest in any year. Peak incidence occurred in 1975, 468 cases per 100, 000 population. The incidence has been declining, to 356, 000 reported cases, 119 per 100, 000 in 2007. The true incidence is propably twice what is reported.
