Neuro Exam: You need a neurologic exam and probably some imaging of your brain; maybe a spinal tap as well. Many neuro. Symptoms brings to mind conditions like, but not limited to, multiple sclerosis. This should be sorted out so you can relax if it's something less worrisome and/or get appropriate treatment.
