Does arthrocentesis in toe hurt?

Generally Not: Generally before any small joint arthrocentesis (tapping of the joint) the skin is numbed. This injection has some minor pain but this is much less than tapping a joint without it and it also allows a larger bore needle to be introduced into the joint for a more successful tap.
