Any condom can fail: Any contraceptive can fail, even sterilization! there really isn't a method that's 100% secure. The best method though is the one that you control and the one that you're most comfortable with. If i were a woman, that'd be a long acting reversible method like an iud or nexplanon. The chance of pregnancy is much lower.

Yes: No birth control is 100% effective. Condoms certianly have obvious failures, such as breaking or falling off, but even with "perfect use"'some women will get pregnant.
