Patient specific: If there is associated stenosis and/or myelopathy, decompression is probably needed; without stenosis, can be treated by range of motion exercise, physical therapy, facet injections, etc.

Anything that Works: Typically conservative care is best, exercise, pt and nsaids. However if not better then consider epidural injections vs. Facet injections.
