Microscopic: All gyri of the brain are thinned, the sulci widened, the lateral ventricles large. On microscopy, many neuritic plaques especially in the hippocampus but also diffusely through the cortex. Neurofibrillary tangles in many neurons. Granulovacuolar degeneration and hirano bodies are common, as is amyloid in vessels. A great many "alzheimer" patients really have something else.

Brain atrophy: Grossly the brain show atrophy. Microscopically there are neurofibrillary tangles hirano bodies and neuritic plaques. However, pathologist utilize a grading scheme to make sure there are more plaques and tangles than expected for normal aging.
