If this is acute: And there is blood in a liquid state - the blood can be drained prior to it solidifying.....Once the blood is dried, if not painful it becomes a waiting game until the nail grows and the material can be cut away.A podiatrist usually can more aggressively trim that nail down than you would be able to.

Bruised toe nail: If there is blood under the nail, it should be drained otherwise just conservative measures such as soaking the toe in epsom salts and warm water will help followed by a band aid and neosporin.
