Many: If you take too much vitamin a during pregnancy many birth defects may occur affecting the heart , spinal cord, cleft palate and lip and something called hydrocephalus where there was too much water on the brain.

No supplement needed: The following link from the world health organization gives some god information about who should take a vitamin a supplement.
