Number of causes: Various disorders can manifest on the lips. Lichen planus, solar keratosis, skin cancer, yeast infection, lichen sclerosis. Just had a patient with this exact description. After three weeks, sent to dermatologist, who biopsies it in his office (the initial local anesthetic tiny needle was the only painful part), and it turned out to be a precancerous, sun-damaged solar keratosis lesion.
