Shoulder pain: A pmr or orthopedic doctor can diagnose this easily. Most likely it is a bursitis , or inflammation of the sac that lubricates the shoulder joint. This will disturb sleep when someone sleeps on it, and radiating pain down the arm is common. Typically it stops above the elbow, so an exam of the wrist to id carpal tunnel, or the neck to rule out a pinched nerve (radiculopathy) from the c spine.
