Where and from what?: If this is from a nail or another object thru your shoe, or a cat bite to the hand you should definitely be seen by a doc. There are countless other scenarios you would need to be seen for, and still countless others that would be no big deal. If you have increasing swelling, pain, redness, drainage, or if the puncture object was dirty or animal- go to ed. May need tetanus/antibiotics/excision.

Have it evaluated: Puncture wounds can lead of severe swelling and infections. These infections could spread deep down to bone. If it gets severe enough, this could lead to loss of limb or even life. With puncture wounds, one may also develop tetanus. Have the wound properly evaluated and treated to avoid preventable complications.
