It is rare in the US: Tetanus has been decreasing in this country since the use of routine vaccination. Rates of disease are higher in people who aren't vaccinated, which is more likely in migrant workers. There are also higher rates in IV drug users. Overall there is about one case of tetanus per 100, 000 population.
