It means that the : Disc at the spine level chest region known as thoracic between the vertebral bones thoracic 4 and 5 has herniated into the spinal canal and is touching or effacing the spinal cord but not compressing it so even though it is significant or large there is still room enough within the spinal canal to accommodate both the spinal cord and disc herniation.
