Additional options: Hoping you have optimized your nutrition also, and are exercising consistently? These are important and often overlooked. Additional treatment options include methylfolate (esp if you happen to have very common variations in certain mthfr genes); augmentation with low-dose antipsychotics like abilify, (aripiprazole) etc; transcranial magnetic stimulation; ect. There are also acupuncture and homeopathy.

Treatment Resistance: Such an incredibly hard position to be in. Can I tell you though please, that everyone responds to treatments differently and that slowly trying every option you can find should prove profitable. I always find when people work methodically with a doctor and try all medication options they end up finding something that is definitely helpful. Hit that sleep hard though first. All the best!
