Severe urinary urgency and frequency, lasting only a few hours and recurring every few weeks. occasional incontinence and hematuria. urine tests/culture are normal, no pain/burning. what could it be?

Gist. Your symptoms are in many ways consistent with overactive bladder syndrome. It's possible that you always have microscopic hematuria.
