Probably surgery: A shoulder injury in twenties male caused a tear of supraspinatus muscle, which goes over top of shoulder to top of humerus, the upper arm bone. Function of this muscle is to elevate arm in motion called abduction. Since tear is at place that tendon of muscle attaches to shoulder musculotendinous cuff, it will likely not heal spontaneously. Surgery to attach tendon will be required.
