How can i get a doctors note saying that i'm "permanently injured from sports" when i'm really not?

No way: A moral person does not ask another person to lie for them. Asking your doctor to come up with a note that you are permanently injured is actually asking your doctor to commit fraud, since such a note could also be used to claim disability. You should get some backbone and do the gym!
