Many things: There are many conditions that have relatively small hands. If the ears are really large and the hands are really small, I suggest a geneticist for evaluation. They should be able to determine if it is part of a syndrome.

Variety of things: There are at least 3 dozen named syndromes that have relatively small hands. Ear size is a bit relative & the addition of generous or prominent ears makess me think of robinow , aarskog, kabuki or prader willi syndrome. There is no way to tell without a full evaluation.
