An insurance issue: Almost always, an insurance company only pays for a part of the cost of a consultation with a doctor. Depending on how much the doctor charges, how much the insurance company paid him, and whether or not the "rules" prevent him from billing the patient the remainder, the patient may end up paying a different amount at different doctors. It's just like paying for any other type of personal service.
