Very ill: If a person cannot keep himself/herself safe as an outpatient due to suicidal thoughts and intents, there would be constant suicide risk. For such a person, dialectical behavior therapy may help -- but it would have to be started in a hospital. Suicide is viewed as part of being mentally ill, so it's assumed that treatment is needed. Depending on the person, it may still not be successful.
