Depends: New or old? One side or both? Any new changes in the breast, nipples included, should be evaluated by a physician and you need to make sure you stay current with your screenings. If it's not related to anything serious but bothers you enough, you may need surgery to fix it. Otherwise do like the beatles and "let it be". Make sure your doc sees it either way, though.
