Immune reaction: The body identifies the foreign red cells and launches attack to destroy "invaders." The cells are destroyed by immune system, proteins/cell components get filtered out by kidneys, but can overload and cause kidney injury--> failure and shock. In sick patient, can be fatal (=why crossmatch process is so intensive).
