Yes, needs eval: Coffee ground emesis desribes vomiting up material that looks like 'coffee grounds' but is actually blood in the stomach. This is a serious problem whether there is blood in the stool or not. Coffee ground emesis needs to be evaluated by a physician immediately as this can be a life-threatening problem.

Yes: Coffee ground emesis is largely made of blood which has been partially broken down in the gastrointestinal tract. It is never normal for this to happen and requires investigation as to the cause. Usually the blood is from the stomach or esophagus, but it requires urgent workup regardless of whether its also in the stool.
