Can it hurt to get a cyst cut open even in painkillers?

Yes: Taking pain killers does not result in not having any pain at all. It may dull the pain experienced, but you may likely receive a local anesthetic to the area before cyst removal and that should result in significant pain reduction and hopefully not much pain while getting it removed. Speak with your doctor about your concerns about pain with the procedure and hopefully they can address this.
