Hard stuff: Spot treatment is going to lead to frustration and disappointment. Get with your personal physician. Your next step may be adding a topical retinoic acid derivative, or moving to benzoyl peroxide over all affecte areas as tolerated plus a short course of an antibiotic, topical spironolactone, or something similar.
