No meds, no relief: You have to take meds to get rid of pinworms. They are not just going to go away.

Hard to say: Pinworms can be regarded as gut pests. They do little other than reside in your gut, laying eggs occasionally in a goo that makes you butt itch. They may disappear periodically, especially if you get diarrhea. You remain a public health risk if you are a carrier, but if you are not bothered by the itch, it could stay forever or clear on its own.
