Treatment for syphilis in 2012. titer went down to 1:1, f/u 6 months later was non-reactive. latest f/u at 1 year was 1:1 again. no new exposure. bad?

No: Rises in titer only count if they're 2-dilution rises. From non-reactive to 1:1 is a 1-dilution rise, and is not felt to be significant. A rise from non-reactive to 1:2 would be significant. However, if you were re-infected with syphilis, your titer would probably be much higher than 1:2.
