Depends : You are 20 days early. Do you usually get bad cramps ? Is this the first time? Have you been sexually active? Could be an infection precipitating early period, is it misscarriage? Many questions need to be answered. Do a pregnancy test if you suspect pregnancy. See a doctor if you suspect an infection. But still could be a normal mishap.
